Student information Sheet

Parents: Print, fill out, and send in this form to update a student's demographics, ethnicity, parent/guardian, emergency, and other information. Required to be updated on an annual basis at the beginning of each school year.

Automatic Notification Update Form

Parents: Print, fill out, and send in this form to update the phone numbers you wish to be contacted at when we use our Automatic Notification System.

PLEASE NOTE: When we receive one of these forms, we will call you back to confirm identity and the numbers to be used.
